Please add the Waiver to your cart then you will be asked to provide information and sign. You will then need to “check out” in order for the Waiver to be processed.

If you are signing a waiver for multiple youth dancers, please complete a separate registration for each dancer. For example, select the waiver, add it to your cart, complete that registration, then return to the registration and repeat that process. Thank you!
